A) Dynojet Research Inc., Paint Room: On or about April 26th, 2023 and at times prior employees were exposed to fire hazards while performing spray finishing operations with flammable paint in a booth that was covered with combustible material.

The employer failed to conduct a task specific hazard evaluation to determine which personal protective equipment was needed to protect the employees.
