29 CFR 1910.28(b)(13)(i): When work on low-slope roofs is performed less than 6 feet (1.6 m) from the roof edge, the employer did not ensure each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system: a) On or about October 15, 2018, Georgia-Pacific Wood Products, LLC, Mt. Jewett plant, roof of MDF building. Employees were not using fall protection while throwing dirty filter bags and other debris off the roof. The employees were exposed to an 85-foot fall while working approximately 1 foot from the edge of the low-slope roof.

Or, when performing work that was both infrequent and temporary, the employer did not use a designated area: a) On or about October 15, 2018, Georgia-Pacific Wood Products, LLC, Mt. Jewett plant, roof of MDF building. Employees were not using fall protection while bagging and removing dirty filter bags and other debris off the roof. Employees were exposed to an 85-foot fall while working approximately 6-10 feet from the edge of the low-slope roof.

29 CFR 1910.30(a)(3): The employer did not train each employee in at least the topics noted in 1910.30(a)(3)(i) through 1910.30(a)(3)(iv): a) On or about October 15, 2018, Georgia-Pacific Wood Products, LLC, Mt. Jewett plant, roof of MDF building. Employees were not trained in the nature of the fall hazards in the work area and how to recognize them or the procedures to be followed to minimize those hazards. Employees were on the roof without fall protection, bagging and removing dirty filter bags and other debris off the roof.
