29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a)  On or about October 9, 2018, the employer did not ensure that employees utilized energy control procedures on the winder prior to conducting paper jams and job set up. The winder was not isolated from the electrical energy source prior to engaging in these activities. Employees were exposed to hazards from the cutter and other moving parts while all hazardous energy sources were not controlled prior to conducting work.      As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], are not implemented to protect employees from machine servicing hazards such as, but not limited to, tool changes and setup operations.
