29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a) 20 Sylvan Road, Woburn, MA 01801 On or about October 16, 2018, work tasks conducted in the chemical receiving area were not assessed for personal protective equipment. Employees are exposed to splash and contact hazards from tasks including but not limited to receiving chemicals where employees are exposed to chemicals including, but not limited to acids, bases, and solvents. Skyworks Solutions, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.132(d)(1)(i), which was contained in OSHA inspection number 1219580, citation number 1, item number 1, and was affirmed as a final order on July 31, 2017, with respect to a workplace located at 20 Sylvan Road, Woburn, MA 01801.
