Northwest Bank - On or about 10/16/18, and at times prior thereto, an electrical outlet box was dangling from the hole in which it was installed. The outlet was not secured into place with screws to prevent it from falling out of the wall hole where it was installed. The outlet was powering a soda vending machine.
