Main Shop: On or about October 18, 2018, and at times prior, employees were exposed to lacerations while cutting metal frame rails and pipe with an unguarded Wells MFG Corp horizontal band saw to cut (metal). b. Main Shop, Bay 18: On or about October 18, 2018, and at times prior, employees were exposed to lacerations hazards while cutting metal frame rails and pipe with an unguarded vertical band saw Rigid No. 970.17021 Serial Number 586. c. Gear Room: On or about October 18, 2018, and at times prior, employees were exposed to struck-by hazards while pressing bearings with an unguarded Red Arrow Hydraulic Press Serial Number S616.

Main Shop, Bay 22: On or about October 18th, 2018, and at times prior, employees were exposed to struck by hazards while performing grinding operations on the abrasive wheel side of a Sioux 6" Ball Bearing Grinder (Serial Number B6683) whose work rest was approximately 1/4" away from the wheel.
