29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    1. Employees performing roofing work on a sloped roof are not protected from fall hazards through the use of a fall protection system.      The employer fails to assure effective protection against falls as required by the standard, as demonstrated on or about October 17, 2018, at 9226 Shadow Oak Lane, Naples, FL, 34120, when employees were applying cement on a sloped roof with a pitch of 6:12, without the use of conventional fall protection, were exposed to a fall hazard of 11-feet to ground level.     a) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), or its equivalent standard, which was contained in OSHA inspection number 1136266, citation number 01, item number 001, with respect to a workplace located at Courtyards Estero Apartments Building 17 Corkscrew, Estero, Florida, 33928 and was affirmed as a final order on 09/12/2016.      b) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1115276, citation number 01, item number 001, with respect to a workplace located at 6622 Everton Ct., Fort Myers, FL 33966 and was affirmed as a final order on 04/01/2016.      c) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), or its equivalent standard, which was contained in OSHA inspection number 1062819, citation number 01, item number 001, with respect to a workplace located at 6650 NW 106 Ave., Miami, FL 33178 and was affirmed as a final order on 11/17/2015.    d) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1269555, citation number 02, item number 001, with respect to a workplace located at 239 Stella Maris Drive S., Naples, FL 34114 and was affirmed as a final order on 07/19/2018.    e) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(10), or its equivalent standard, which was contained in OSHA inspection number 1242436, citation number 01, item number 001, with respect to a workplace located at Cypress Ridge Blvd, West Chapel, FL 335444 and was affirmed as a final order on 09/20/2018.    f) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1233510, citation number 01, item number 001, with respect to a workplace located at 4952 Kyngs Heath Road, Kissimmee, FL 34746 and was affirmed as a final order on 09/20/2018.    g) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1044419, citation number 01, item number 001, with respect to a workplace located at 1345 Shinnecock Hills Drive, Davenport, FL 33896 and was affirmed as a final order on 04/03/2015.    h) Crown Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), or its equivalent standard, which was contained in OSHA inspection number 1028907, citation number 02, item number 001, with respect to a workplace located at 10525 NW 66th Street, Doral, FL 33172 and was affirmed as a final order on 01/19/2016.
