29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: Location: Roof located at 160 Hobart Ave, Braintree MA Employees were exposed to fall hazards of approximately 13 feet from porch roof to ground, while working on an 8 pitch slate roof on a residential home, where fall protection was not used properly.
