29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about 16 October 2018, at the subject jobsite, employees were not provided or required to wear hard hats in a demolition zone.  Employees were at different levels of height, performing demolition, while other employees were below picking up the debris and disposing of it. Employees were exposed to head injuries from falling debris.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 16 October 2018, at the subject jobsite, employees were observed working at heights in excess of 11 feet. Employees were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems while performing demolition work.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed.  On or about 16 October 2018, at the subject jobsite, written evidence was not available that an engineering survey was performed by a competent person prior to permitting employee(s) to start demolition operations. Employees were exposed to possible hazards that could have been identified from a properly conducted engineering survey.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
