29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a) On the south side of the roof:  On or about 10/19/2018, an employee working on a 6/12 pitched roof was not protected from falling approximately 14' 1".

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:    (a) Jobsite:  On or about 10/19/2018, two extension ladders and one step ladder were not inspected by a competent person, in that they were being used with split side rails, a damaged rung and cracks around the locking mechanism.
