29 CFR 1910.25(b)(1): The employer did not ensure handrails, stair rail systems, and guardrail systems were provided in accordance with 1910.28:   Shot Reclamation Platform:  On, or about, November 8, 2018, the fixed stairs that provided access from the ground level to the first level was not provided with a midrail on the open side and a handrail on the closed side in accordance with Table D-2 as referenced by in 29 CFR 1910.28(b)(11)(ii).     Abatement Note:  29 CFR 1910.29 provides specific design criteria for guardrail and handrail systems.

Shot Reclamation Platform:  On, or about, November 8, 2018, the access stairs from the ground level to the first level were measured to be 20 inches wide.  The supports for the vertical opening were slightly narrower than the stairs.

The dip tank contained MIL-PRF-680, Type II, a category 4 flammable liquid.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval and capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:     Facility:  On, or about, October 18, 2014, a barrel attachment was used with the CAT forktruck, serial number AT9020559, to move barrels of steel shot in the plant.  The forktruck had not been evaluated and marked to show that the barrel lifter could be used and the reduced capacity of the forktruck when it was used.  The estimated reduced capacity of the forktruck was 1,483 pounds.  The weight of a full barrel of shot was estimated to be 1,837 pounds.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    Company:  On, or about, November 8, 2018, an employee who performed maintenance or and around electrical equipment had not been trained regarding what work practices and protective equipment were required to protect him against electric shock, arc flash and arc blast hazards while performing live  trouble-shooting and when verifying whether or not electrical equipment was properly de-energized.    Abatement Note:  At the time of the inspection, some flash gear could not be located.  Ensure that personal protective equipment such as a helmet, face shield and fire resistive balaclava are provided for use by employees.
