The employer does not protect each employee by means of a guardrail system, safety net system, personal fall arrest system, or an alternative fall protection measure. The following instances most recently occurred on October 23, 2018, at the above addressed jobsite: Employees engaged in commercial roofing operations were not protected from a fall, exposing the employee to a fall of approximately 12 feet. The A & E Roofing & Siding, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1318955, citation number 2, item number 1 and was affirmed as a final order on August 9, 2018, with respect to a workplace located at 2425 Barrington Road Hoffman Estates, IL 60010. The A & E Roofing & Siding, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1209451, citation number 2, item number 1 and was affirmed as a final order on April 26, 2017, with respect to a workplace located at 1025 Douglas Ave. Naperville, IL 60540. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
