29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:   a) Job site, 7223 McKnight Road, Pittsburgh, PA: On or about October 23, 2018, during construction activities, the employer did not ensure that an employee was using appropriate fall protection systems/devices while in the basket mounted on the forks of a forklift being lowered to ground level, where the employee was exposed to a fall hazard of at least approximately 22 feet.  b) Job site, 7223 McKnight Road, Pittsburgh, PA: On or about October 23, 2018, during construction activities, the employer did not ensure that an employee was using appropriate fall protection systems/devices while in the basket mounted on the forks of a forklift while installing plywood at the top of the exterior of the building, where the employee was exposed to a fall hazard of at least approximately 17 feet.
