Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to pinch point and amputation hazards when removing membrane elements on a reverse osmosis system: Location: Bottling facility, Reverse Osmosis System #3, #2 Vessel On or about October 19, 2018, employees were exposed to amputation hazards while removing membrane elements. An employee placed their hand into vessel #2, while another employee on the opposite end of the vessel pushed the membrane element out.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: On or about 10/23/2018, an employee operated a Nissan 60 powered industrial truck in the hallway outside the filler room to transport a pallet of plastic bottle caps and did not keep a clear view of the path of travel.
