29 CFR 1910.147(c)(6)(i) - The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Location - Throughout Facility On or about 04/30/24, the employer had not conducted a review of the lockout/tagout program at least annually. The written lockout/tagout program was last revised October 12, 2022 and the machine specific procedures for the L6 Palletizer document was last revised May 19, 2022.

29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety: a) Location - Throughout Facility On or about 04/30/24, unqualified maintenance personnel had not been trained in safe work practices and protective equipment was required to protect them against electric shock, arc flash and arc blast hazards while working on or around live electrical equipment.
