OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  On or about 10/23/2018 at the routing machine area, employees were exposed to struck by hazards where the employer failed to provide adequate lifting adapters to employees performing lifting materials to heights up to approximately 4-feet, including but not limited to up to 350 pound cams. The employer provided employees with "C" clamps that were not designed for this application.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 10/23/2018, throughout the facility, workers performing operations including but not limited to cleaning and set-up were exposed to the release of hazardous energies where the employer failed to perform annual inspection of the machine specific, written procedures since creation on 5/5/2016.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  On or about 10/23/2018, at the CNC machine, workers were observed exposed to struck by hazards where the employer allowed the use of the Toyota powered industrial truck S/N 20914 to suspend loads (Free rigging) from the forks to load and unload the cams onto and off of the CNC machine.

29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer:  On or about 10/23/2018, throughout the facility, workers using slings to lift cams weighing up to 600 pounds, were observed exposed to struck by hazards where the employer failed to require daily inspections of slings and prohibit the use of damaged slings to lift equipment including but not limited to cams weighing up to 600 pounds.
