29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Location: Fabrication Area When employees were servicing or maintaining electrical equipment used to fabricate exterior wall systems, the employer did not establish an energy control program to protect the employees from injuries related to the unexpected energizing, start-up, or release of stored energy. Such equipment included: Rollers, shapers, shears, brakes, and CNC machines

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed so as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Location: Fabrication Shop Employees were exposed to laceration and crush hazards while operating the Primeline shear where the guard for the blade and the hold downs was inadequate in that it contained a 40 inch long hinged section that could be opened during operation.
