On or about October 23, 2018, at the above addressed jobsite, the employer did not maintain an accident prevention program to comply with the regulations of this part which included, but were not limited to, employee protection when exposed to hazards related to commercial construction.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      a.  On or about October 23, 2018, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.

On or about October 23, 2018, at the above addressed jobsite, an employee using the extension ladder carrying roofing material while accessing the ladder could have lost his balance, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a.  On or about October 23, 2018, at the above addressed jobsite, employees conducting roofing operations, approximately fourteen (14') feet, three (3") inches above the ground, were not protected from falling.    Dardon Construction Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1270694, citation number 02, item number 01 and was affirmed as a final order on February 01, 2018, with respect to a workplace located at 569 Arlington Ave., Des Plaines, IL.    Dardon Construction Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1012144, citation number 01, item number 02 and was affirmed as a final order on March 13, 2015, with respect to a workplace located at 1105 S. Wesley Ave., Oak Park, IL.
