29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about June 11, 2019, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about June 11, 2019, at the above addressed jobsite, employees performing residential roofing operations, seventeen (17) feet two (2) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Dardon Construction Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1355677, citation number 2, item number 1 and was affirmed as a final order on February 11, 2019 , with respect to a workplace located at 35 S. Paulina St., Chicago, IL.  Dardon Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1270694, citation number 02, item number 01 and was affirmed as a final order on February 01, 2018, with respect to a workplace located at 569 Arlington Ave., Des Plaines.  Dardon Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in osha inspection number 1012144, citation number 01, item number 02 and was affirmed as a final order on March 13, 2015, with respect to a workplace located at 1105 S. Wesley Ave., Oak Park, IL.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about June 11, 2019, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
