Machines were operated without the use of guards to prevent the employees' body from coming into contact with the danger zone. Employees were exposed to entanglement hazards.

Employer did not securely attach the upper guards to prevent the employees from coming into possible contact with the danger zone. Employees were exposed to amputation hazards.
