29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about October 24, 2019, at the shredder vibrator located at 4500 Oaks Rd., Davie, Florida, an employee who performed replacement of a protective cover for pulleys and belt of a the shredder vibrator did not receive specific training in the procedures of securing energy to the equipment. On or about October 24, 2019, at the shredder vibrator at 4500 Oaks Rd., Davie, Florida, an employee who performed electrical wiring work on an electrical junction box did not receive specific training in the procedures of securing energy to the shredder vibrator. NOTE: CERTIFICATION AND ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS CITATION.
