Davie, FL 33314, the employer did not implement an effective hearing conservation program for employee who was exposed to noise levels of 367.3% dose or an equivalent sound level of 99.3 dBA (over the Action level of 8 hour time weighed average sound level of 85 dBA). The sampling was performed for 476 minutes.
