29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     At worksite located at 900 NW Hunter Dr, Blue Springs, Missouri. Employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the forklift operators had not completed a competent training program.  The employer had  not provided training that consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace for employees who are required to operate a York Serial # A809N04891U fork truck.

Employees were exposed to serious injuries to their hands and fingers, in that the employer did not ensure that the Powermatic brand table saw serial number 9166121B   was provided with a spreader device to prevent material from squeezing the saw blade or from throwing material back at the operator.

Employees were exposed to serious injuries to their hands and fingers, in that the employer did not ensure that the Powermatic brand table saw serial number 9166121B  was provided with an Anti Kick-Back Device to prevent materials from being thrust and thrown back at the operator.
