29 CFR 1910.305(a)(2): Temporary electrical power and lighting installations of 600 volts, nominal, or less were used for purposes other than those permitted in subparagraphs (a), (b) and (c) of this paragraph: At a worksite located at 900 NW Hunter Dr., Blue Springs, MO: Employees were exposed to electrical shock hazards. A two receptacle extension cord was in operations and had bee permanently affixed to a wall in the shop. The extension cord was being used to power a Maxx Air portable fan for a duration in excess of what is approved by this regulation.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: At a worksite located at 900 NW Hunter Dr., Blue Springs, MO: Employees were exposed to electric shock hazards. Employees were working in an area of the shop where portable fans were in use. One of the Maxx Air portable fans was in use and was pulled away from the strain relief device thereby increasing the risk of the conductor wires separating from the terminal.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): At a worksite located at 900 NW Hunter Dr., Blue Springs, MO: Employees were exposed to electric shock hazards. Employees were working in an area of the shop where portable fans were in use. One of the Maxx Air portable fans was in use and had not been inspected for any external defects. The cord showed a section that was damaged and had been covered by electrical tape.

29 CFR 1910.334(a)(2)(iii): 29 CFR 1910.334(a)(2)(iii): Where a connection plug is to be connected to a receptacle, the ratio of the plug and receptacle contacts was not checked first to ensure that the plug and receptacle contacts were of the appropriate coupling settings: At a worksite located at 900 NW Hunter Dr., Blue Springs, MO: Employees were exposed to electric shock hazards. Employees were working in an area of the shop where portable fans were in use. One of the Maxx Air portable fans was in use and plugged into an extension cord that only had two receptacle contacts. The cord equipment had three receptacle contacts and the ground pin was bypassed to allow for the connection to take place.
