29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  At this facility, employees were exposed to corrosive materials and aluminum dust while manufacturing aluminum parts, and were not provided with an eye wash station or other suitable means of quick drenching.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  At this facility, employees were exposed to electrical hazards when utilizing extension cords and/or relocatable power taps to power equipment in the following instances:  a.  AD #6, a Q Standard industrial fan was plugged into a relocatable power tap that was plugged into an extension cord which was plugged into the fixed electrical outlet; and,  b.  Tool room, a small hoist was plugged into extension cord which was plugged into the fixed electrical outlet.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  At this facility, in the end shield die cast (ESDC) areas, employees were exposed to electrical hazards in the following instances:  a.  Damaged extension cord used to operate the Q Standard industrial floor fan; and,  b.  The plug for the Q Standard industrial floor fan was missing the ground prong.

Square D safety switch mounted on the Lodge & Shipley Blue Chip with serial number 27081;  c.  480 volt electrical box with a ML placard on the front panel; and, d.  Junction box adjacent to PW #2-FUG.
