29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part.   A) Mark Yoder on or about October 11, 2018 at the address of 36456 Capri, North Ridgeville, Ohio: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.  B) Mark Yoder on or about October 11, 2018 at the address of 36441 Capri, North Ridgeville, Ohio: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.   A) Mark Yoder on or about October 11, 2018 at the address of 36456 Capri, North Ridgeville, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. The employees did not recognize the hazards associated with working from an elevated location  (12 feet )without the benefit of fall protection.     B) Mark Yoder on or about October 11, 2018 at the address of 36441 Capri, North Ridgeville, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  A) Mark Yoder on or about October 11, 2018 at the address of 36456 Capri, North Ridgeville, Ohio: The employer did not ensure employees were protected from falls while installing new roofing materials (asphalt shingles) to a residential type structure.  The employees were exposed to a potential 12 foot fall to the level below.            B) Mark Yoder on or about October 11, 2018 at the address of 36441 Capri, North Ridgeville, Ohio: The employer did not ensure employees were protected from falls while installing new roofing materials (roofing felt) to a residential type structure.  The employees were exposed to a potential 12 foot fall to the level below.
