29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift. At a construction worksite located at 1405 E McKinsey, Moberly, Missouri. Employees were exposed to engulfment hazards and serious injuries, in that the competent person did not remove employees from hazardous conditions relating to excavation activities when there was evidence of possible cave-in conditions and hazardous atmospheres. An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift.

Employees were exposed to serious struck by and or engulfment injuries, in that there was no protective systems in use for excavation work being done in a trench approximately 10 feet deep.
