29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed.  Domiciliary, on or about 30 October 2018 and times prior thereto; the employer did not ensure that the exit routes were clear and unobstructed.  This condition exposed employees to a tripping and smoke inhalation hazard.      a) second floor stairwell; boxes were stored on the stairs leading to the exit route.       b) mechanical room; miscellaneous items were stored in the exit path leading from the mechanical room to the stairwell.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  Domiciliary, computer station in common area; on or about 30 October 2018 and times prior thereto; a relocatable power tap was connected in series (daisy chained) to a second relocatable power tap. Power source was used to power a computer and monitor. This condition exposed employees to an electrical fire hazard.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows, or similar openings.  Domiciliary, throughout the administrative wings; on or about 30 October 2018 and times prior thereto; energized electrical cords were routed through doorway openings. White noise devices were position outside closed doors creating pinch points on the electrical cords. This condition exposed employees to electrical shock and electrical burn hazards.
