29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:    (a) 48 E Cherry Street - Four employees conducting roofing operations on a steep residential roof approximately 35 feet above ground were not protected by guardrail systems, safety net system or personal fall arrest systems, on or before October 30, 2018.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) 48 E. Cherry Street - Employees using a 32 foot extension ladder to gain access to the upper roof did not ensure that the ladder extended 3 feet above the landing area, on or before October 30, 2018.     No abatement certification or documentation required.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) 48 E Cherry Street - An employee cleaning up debris on the ground was exposed to struck by hazards from debris including but not limited too shingles, roofing paper and plastic being thrown off the roof.  The employer did not ensure that the employee wore a protective helmet, on or before October 30, 2018.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
