29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) 10215 Cummings House Road - An employee cleaning up debris was exposed to struck by hazards from debris including but not limited too: shingles, roofing paper and plastic being thrown off the roof. The employer did not ensure that the employee wore a protective helmet, on or about January 1, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 12015 Cunning House Road - Employees using a 25 foot extension ladder to gain access to the roof to install new shingles and the foreman did not ensure that the ladder extended 3 feet above the landing area, on or before January 2, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) 12015 Cunning House Road - The employer did not tag out a ladder when the right foot of the ladder was defective, in that the foot was missing the rubber padding and a potion of the metal foot, on or before January 2, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
