29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.    The employer is failing to ensure that all employees are protected from hazards involving injury to the eyes when operating pneumatic nail gun. This event was most recently documented at a jobsite located at or about 16470 Vane Street, Bennington, Nebraska, 68007 on October 29, 2018. An employee was working on the roof of a new residential structure, operating a pneumatic nail gun without the use of an appropriate eye protection device.

This event was most recently documented at a jobsite located at or about 16470 Vane Street, Bennington, NE 68007 on October 29, 2018 when employees were utilizing an electrical outlet box (designed to be mounted in or on a wall) as part of an extension cord.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    The employer is failing to ensure that all employees are protected from electric shock or fire hazard when utilizing flexible cords with damaged strain relief fixture connected to the connector itself. This event was most recently documented at a jobsite located at or about 16470 Vane Street, Bennington, NE 68007 on October 29, 2018. Three employees were using a flexible cord with damaged stain relief, exposing other inner electrical cables.

The employer is failing to ensure that all employees are protected from fall hazards when using fork-lifts with scaffold platforms that are not securely attached to the fork, preventing it from moving horizontally while the platform is occupied. This event most recently documented at the jobsite located at 16470 Vane Street, Bennington, NE 68007 on October 29, 2018. All employees were exposed to falls of over 12 feet to the concrete driveway below.

The pitch of the roof was 6:12.
