29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Top and Bottom Services, LLC Located at 1421 Soda Creek Court, Fort Collins, CO 80526: On and Preceding 10/31/2018, the exposing employer did not ensure that employees engaged in re-roofing of a two story residential home were protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Four roofers were working at levels between 10 and 27 feet removing and preparing to install roofing shingles without the use of fall protection. This condition exposed employees to falls up to 27 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Top and Bottom Services, LLC Located at 1421 Soda Creek Court, Fort Collins, CO 80526: On and Preceding 10/31/2018, the exposing employer did not ensure that employees utilizing extension ladders to access the first and second levels of a roof on a two story residential home had the ladder extended at least 3 feet above the upper landing surface. Four roofers utilizing the ladders for access were removing shingles and carrying materials up the extension ladders preparing for installation of the shingles and other materials. This condition exposed employees to falls from height.
