OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards:   a) 89 Maskells Mill Road, Salem NJ, Employees were removing the lowest two courses of a masonry block wall and replacing them with new masonry blocks. They did not brace the masonry blocks that were above those courses. They were exposed to being struck by the upper portions of the wall. An employee was struck and killed when the masonry block wall, above where he was working, broke free and fell onto him. Occurred on or about 10/31/2018.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT  DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
