29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in gallon blow mold machining activities at the following pieces of equipment around the blow mold line areas were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized: a) Unilloy gallon blow mold machine, the hydraulic moving part was not guarded at the point of operation. The procedure was not in place to prevent employees to reach hands inside the machine to adjust for the timing and the flowing of the gallon jugs for proper trimming process. b) Trimmer bed, the electrical interlock on the door had been bypassed to open the safety doors so that employees can reach into the trimming bed area to unjam the gallon jugs which stuck in the trimmer. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
