29 CFR 1910.22(a)(3): Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.      a) Crystal Room: On or before November 1, 2018, and at times prior, the employer exposed employees to slips and falls caused by wet floors.  The crystal room area had very slick floors due to water from numerous leaks in the ceiling.    b) Warehouse Area: On or before November 1, 2018, and at times prior, the employer exposed employees to slips and falls caused by wet and soapy floors. The warehouse areas had slick floors from water due to numerous leaks in the ceiling.

Pallets and super sacks of rework soap were haphazardly placed approximately 15.75" in front of the electrical panel.
