Employer is failing to ensure that employees use eye and/or face protection during residential roofing activities. On November 1, 2018 at or near 16922 Potter St. - Bennington, Nebraska, employees were observed using nail guns and were not wearing appropriate eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  At or near 16922 Potter St. Bennington, Nebraska - Employer is failing to ensure that employees engaged in residential roofing activities at heights above six (6) feet were protected by any type of fall protection device or system. On or about November 1, 2018, four workers were observed installing shingles at heights exceeding ten (10) feet without any type of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At or near 16922 Potter St. - Bennington, Nebraska - Employer is failing to ensure that ladders used by employees to access a roof extended at least three (3) feet above the roof landing surface. On or about November 1, 2018, three extension ladders extended less than two (2) feet above the roof landing access surface area.

On or about November 1, 2018, employees using inadequately extended ladders had not received proper training.
