29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on October 3, 2019 on the jobsite located at8704 Harney St., Omaha, NE 68114. There were 4 employees re-roofing an existing single-story residential house and not utilizing any form of fall protection. The house has a 6 in 12 steep sloped pitch and ground to eve height is approximately 9 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This most recently occurred on October 3, 2019 on the jobsite located at 8704 Harney St., Omaha, NE 68114. Employees are using an aluminum extension ladder to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about 9 feet above the ground.
