29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet:  a) At the jobsite, on or about 11/5/18:  Workers were using a 4 foot Werner step ladder to access the elevated foundation where that ladder did not extend at least 3 feet above the upper landing.  b) At the jobsite, on or about 11/5/18:  Workers were using a 6 foot Michigan step ladder to access the elevated foundation where that ladder did not extend at least 3 feet above the upper landing.
