29 CFR 1910.147(d)(2):      Machine or equipment shutdown.  The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.    On or about 11/05/2018, employees at Bodine Aluminum, Inc. failed to de-energize the BZE-259 Casting Finishing Line prior to performing service on the equipment/line.  Specifically, the employees failed to control the gravity energy source by installing the fall prevention pins.
