29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) 5 South Shaver Street, Mount Union, PA - Employee(s) were observed performing roofing work activity on an 8 in 12 pitch residential roof without the benefit of a fall protection system, thereby exposing employees to a fall from the main roof of approximately 10 feet to the porch roof and 18 feet from the edge of the roof to the ground below, on or about November 7, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.452(k)(5): Scaffold platforms shall not be bridged one to another: a) 5 South Shaver Street, Mount Union, PA - The ladder jack scaffold was erected using four ladders on top of the porch roof, the stage/planking was bridged together using 2x4 in the handle end of the stage/planking to hold the two butted ends together for employees to work from to install roofing on a residential home on or about November 7, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
