On or about November 1, 2018, in the Warehouse Area of the facility located at 1900 Ikea Way in Columbus, Ohio, employees were installing pallet support bars on the top level of pallet racks that were approximately 30 feet tall. The employer never initiated or maintained safety and health programs to protect employees from the hazards associated with assembling pallet racks or working from elevated work surfaces.

The employer never instructed the employees in the recognition and avoidance of unsafe conditions with the regulations applicable to protect employees from the hazards associated with assembling pallet racks or working from elevated work surfaces.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about November 1, 2018, in the Warehouse Area of the facility located at 1900 Ikea Way in Columbus, Ohio, employees were installing pallet support bars on the top level of pallet racks that were approximately 30 feet tall. Employees were not protected from falls while moving their fall harness' lanyard from one tie-off location to another.
