29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about November 8, 2018; roof area of new edition; employees were walking the top-plate without fall protection in place to protect them from falling 30-feet to ground level.                         b) On or about November 8, 2018; second floor; employees were working near floor openings such as but not limited to a stair opening that was not guarded, exposing them to a 10-foot fall to the lower level.    c) On or about November 8, 2018; second-floor area; employees were working near unguarded wall openings, exposing them to a 20-foot fall to ground level; while framing.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a) On or about November 8, 2018; Second-floor area; Employees used  an 8-foot step ladder to access the roof/top plate, which was 4-feet below the platform, exposing employees to a potential 10-foot fall to the lower wood floor or 20-foot fall to ground level.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:   a) On or about November 8, 2018; second-floor area; employees used an 8- foot step-ladder, which was too short, to access the roof/top-plate area, located 10-feet above the floor, exposing them to a fall hazard when using the top-step of the  ladder.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
