29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.    a) Second Level Center and Edge of Structure - Worker cutting wood studs to be used for framing the second level walls, was not wearing eye protection to prevent flying materials, wood chips and saw dust from entering and injuring his eyes, on or about, May 7, 2019.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Second Level of Structure  - Employees were constructing the wood stud wall frames were not protected from falling approximately 15 feet to the ground below, on or about 05/07/2019.    No abatement certification or documentation required for this item.    Olson Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1359176, citation number 1, item number 1 and was affirmed as a final order on 02/27/2019, with respect to a workplace located at 22109 Foxhall Rd NW, Washington, DC 20016.
