On or about Friday, November 9, 2018 at 21-32 Borden Avenue, Long Island City, NY 11101 A 2nd floor mezzanine level accessed by employees to retrieve merchandise was observed missing a guardrail system exposing employees to a fall hazard of approximately nine (9) feet to the lower level. ABATEMENT DOCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
