29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which were not supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate.   At the break room area, the employer stored materials above the office without a load rating or a sign indicating the approved load limit exposing employees to struck-by or crushed-by hazards.

29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level.   At the warehouse shipping office, the stairway had a guardrail that consisted of a top rail and post but did not have the intermediate rail exposing employees to fall hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed.     In the re-roll machine area, there was not a guard covering the 19 inch chain, and the 1/2 inch and 5 1/2 sprockets on the re-roll machine exposing employees to hazards of being caught-in chain and sprockets.

In the shop areas, the employer had electrical panel boxes that energize lights and machinery which were not marked with voltage ratings:  a)  In the warehouse area, panel box east side. b)  In the shipping area, panel box west side.
