29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: In the fabrication shop area, the employees used a Milwaukee chop saw to cut metal material with the lower guard tied in the up position, preventing the guard from automatically returning to the lower position to cover the saw blade. Employees were exposed to struck-by and amputation hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. On the west side of the warehouse, the employer did not ensure the employees had clear access to the fire extinguisher at all times. The employees were exposed to fire and smoke hazards.
