29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board:  (a) The employer did not ensure employees were using a method of fall protection, grab line or equivalent hand hold when conducting roofing work from a crawling board.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) The employer did not ensure employees were using fall protection when loading and unloading roofing equipment and material from different roof levels. The employees were briefly located on lower and upper flat roofs with parapet walls less than 39 inches.

The employees were using crawling boards that were shorter than the length between the eaves and roof peak.
