(a) On or about January 03, 2020, the tube and coupler type scaffold erected on the South side of the construction site was not fully planked on the top work level, exposing employees to a fall hazard and struck by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about January 03, 2020, employee(s) installing ceramic tile roofing on the South side of the structure and near the south rake edge of the roof were not protected from falling to the lower level. This exposed employee(s) to a fall hazard of approximately Forty (40) feet. The Millen Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(iii) 29 CFR1926.501(b)(1), which was contained in OSHA inspection number 1359433, citation number 1 item number 1 and was affirmed as a final order on 01/11/19, with respect to a workplace location at 631 N. 19th St., Milwaukee WI. 53224.

29 CFR 1926.405(a)(2)(ii)(E): All lamps for general illumination shall be protected from accidental contact or breakage. Metal-case sockets shall be grounded. (a) On or about January 03, 2020, in the walkway area under the scaffolding, the 4th light from the end of the string, the bulb cage was open and not properly secured, exposing employees to hazards related to accidental bulb breakage, and contact with exposed electrical current.
