29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, items A through D of this section:       a) Production area, on or about November 9, 2018, a specific energy control procedures (lockout tagout procedure) were not developed and documented for the machines that included but were not limited to the press brake, plasma and laser tables. The only procedure for the machines was a generic procedure that did not contain the specific methods, techniques, and procedural elements outlined in 29 CFR 1910.147(c)(4)(ii)(A) through 29 CFR 1910.147(c)(4)(ii)(D)

Annual periodic inspections of the lockout/tagout procedures (energy control procedures) had not been conducted.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:     a.) Venango Steel, Inc. in Franklin, PA.  On or about November 9, 2018, fork extensions were utilized on the Hyster forklift truck.  Approval from the manufacturer for the forklift attachment was not obtained.  In addition, the capacity plate on the forklift was not modified to reflect the effects on the capacity of the forklift when the attachment was used.

Venango Steel, Inc. in Franklin, PA.  On or about November 9, 2018, fork extensions were utilized on the Hyster forklift truck. The Hyster forklift remained in operation with damaged tires.
