29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling: (a) Facility, 2900 South Noah Drive, Saxonburg, PA: On or about November 13, 2018 employees working at the pre-stressing bed was not protected from falling approximately 70 inches from the top of the bed to the floor below by guardrail systems, safety net systems or personal fall arrest systems. Employees walk along the top of the bed that is approximately 18 inches wide to access the work area or during the pouring process exposing them to fall hazards.
